Senior Care Partners PACE, is a Program of All-Inclusive Care for the Elderly (P.A.C.E.) model of care. P.A.C.E. is an innovative program in which we assist elders to live as independently as possible with the team support of healthcare professionals. P.A.C.E. provides health care services for frail elderly individuals who are eligible for the nursing home level of care in the comfort of their own home in the community. Senior Care Partners P.A.C.E. coordinates and provides all needed preventative, primary, acute and long-term care services.
Primary Purpose
Provide nutrition care and education to Senior Care Partners P.A.C.E. participants. This includes evaluating the participant’s nutrition status, developing and implementing nutrition care plans, assessing nutrient requirements, and evaluating the effectiveness of nutrition care. Responsible for communicating nutrition status to the physicians and Interdisciplinary Team (IDT) members.

  • Assess, develop and implement care plans.
  • Monitor meals in day center and in the home.
  • Manage therapeutic and special diets.
  • Monitor key clinical indicators of nutrition status.
  • Manage Nutrition Support.
Duties and Responsibilities
  • Conducts initial and periodic assessments, identifies those at nutrition risk, and develops a specific plan of care for the nutrition needs of participants per policy. Reassess every 6 months as needed.
  • Functions as a member of the IDT, communicates participant changes to team members, participate in IDT meetings, and the development of the participant plan of care.
  • Conducts home visits if a need is identified.
  • Provides nutrition education/consulting with participant/family regarding special dietary regimens and nutrition requirements by referral or Academy of Nutrition and Dietetics standards.
  • Acts as a resource to other staff regarding nutrition requirements and education.
  • Determines the most appropriate nutrition interventions to meet a participant's need and monitors the effectiveness of these interventions.
  • Communicates with food service vendors as needed regarding the menu, daily lunch orders, weekly food supply orders, etc.
  • Participates in the coordination of the home nutrition services such as take-home meals, supplements, Meals on Wheels, etc., and maintains the participant's schedule in the Electronic Medical Record system.
  • Assists in monitoring meals, including intake, diet compliance, and tray accuracy.
  • Monitors participant's chewing/swallowing function, initiation of meals, and ability to self-feed (collaborate with other disciplines as needed, SLP/OT, etc.).
  • Responsible for the management of therapeutic diets, including texture modifications, food allergies, and other diet restrictions to meet individual participant needs and comply with physician orders.
  • Maintains sanitation of food preparation and kitchen area through annual inspections and as needed.
  • Refers participants to appropriate community agencies or facilities and acts as a liaison/participant advocate with such organizations.
  • Participates in developing and maintaining internal Nutrition Service policies.
  • Collaborates routinely with outside facilities such as dialysis, nursing homes (NH), skilled nursing facilities (SNF), assisted living facilities (ALF), adult foster cares (AFC), boarding homes, etc.
  • Collaborates with IDT and the home enteral/parenteral infusion companies to manage participants' home nutrition support needs.
  • Mentor dietetic interns and medical students regularly throughout the year.
  • Other duties as assigned.
Supervisory Responsibilities
This position has no supervisory responsibilities.
Qualifications
  • Bachelor’s Degree from an Academy of Nutrition and Dietetics accredited institution with a major in food and nutrition or dietetics; Master’s Degree preferred.
  • Completion of an accredited Dietetic internship with a minimum of 1,200 hours of supervised practice hours.
  • Minimum of one year of experience working with the frail, elderly, or long-term care population.
Specific Requirements
  • Possess current CPR certification by AHA or willing and able to obtain as offered by Senior Care Partners P.A.C.E.
  • Possess current ServSafe Manager Certification or willing and able to obtain as offered by Senior Care Partners P.A.C.E.

  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to plan, organize, develop, implement, and interpret Senior Care Partners P.A.C.E. goals, objectives, policies and procedures.
  • Must be able to maintain confidentiality regarding sensitive and private information.
  • Immunizations and vaccines must be current or medical documentation must be provided from a medical provider with reason (s) to request waiver (Hepatitis B, TB, MMR, Varicella, TDaP, Flu, COVID-19).
  • Must have reliable transportation and be able to travel between centers as needed; current valid Michigan driver’s license and current valid vehicle insurance required.
Work Environment
  • Has visible presence while working in the center and is subject to frequent interruptions.
  • Communicates with staff, participants, caregivers, and other organizations as necessary.
  • Works beyond normal working hours and on weekends and holidays as necessary.
  • May be subject to exposure and handling of infectious waste, diseases, conditions, or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or Hepatitis B virus.
  • Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
  • Required to drive a car in all types of weather.
Physical Demands (With or without accommodations)
  • Must be able to cope with the mental and emotional stress of the position.
  • Must function independently, and have flexibility, personal integrity, and the ability to work effectively with participants, staff, and support agencies.
  • Must be able to push, pull, move, sit or stand for long periods of time.
  • Lift up to 20 pounds as needed.
Benefits
We offer a competitive benefits package including medical, dental, vision, Teledoc, HelpNet, and company-paid life insurance and AD&D at $50,000 each, with the option to purchase additional voluntary insurances – STD, LTD, AI, CI, and additional life and AD&D. Our employees are eligible the first of the month following 30 days of service. All salaried staff receives 20 days of PTO accrued during the year, and non-exempt staff receives 18 days, which is accrued right away. Employees may start contributing to our 403(b) immediately, and after one year of service the company will match up to 3%. After 90 days of employment full-time employees are eligible for an 8-hour floating holiday, and receive one each calendar year thereafter. All full-time employees receive the six major paid holidays.
